Note:
Death records from 2022 to present are available Statewide at any municipality. Prior to 2022, you may obtain the record from Pawtucket if the death occurred in Pawtucket or the person was residing in Pawtucket at the time of their death.

Where do I obtain a copy of a death certificate?

Order a Death Certificate in Person:

Present a valid photo ID of yourself, such as a Drivers License, State ID or Passport

Order a Death Certificate by Mail

Send a written request by mail:
State the decedent’s full name, date of death, and where it occurred.

Enclose a copy of your valid photo ID, such as a Drivers License, State ID or Passport.

Enclose check or money order made payable to “City of Pawtucket'”

What do I need to obtain a copy of a death certificate?

If arriving in person, present a picture identification of yourself.

If mailing a request, please fill out the following form:

What is the cost of a death certificate copy?

In person:

$22 for the first certificate

$18 for each additional certificate, of the same record, when
ordered simultaneously.

By mail or online:

$25 for the first certificate

$18 for each additional certificate, of the same record, when
ordered simultaneously.

Note:
Online requests will have an additional $5 surcharge when using a credit card.
