Copies of FIRE REPORTS can be obtained by filing a request for public records through the city’s Law Department. Please click here for the request form. All requests are handled in accordance with the Access to Public Records Act (APRA).
Copies of EMS/RESCUE REPORTS can be obtained by one of the following:
– Patients can request a copy in person at fire department headquarters when presenting a valid photo ID.
– Attorneys on behalf of the patient with a medical prior authorization signed by the patient fax request to 401-205-1687
– Persons on behalf of a deceased patient legally designated as an heir at law in person with valid paperwork and a medical authorization form. Click here to download the form.
